Disease management programmes
Disease management programmes (DMP/Curaplan) are structured treatment programmes for chronically ill patients that are supported by the statutory health insurance funds. DMPs are intended to contribute to improving the treatment of chronic diseases over their entire course through targeted care management in the form of standardised treatment and care processes. They are intended to alleviate impairments caused by the disease and reduce secondary diseases.
The aim is to coordinate treatment across the boundaries of the individual service providers, to ensure needs-based and economic care and to reduce existing deficiencies in care (overuse, underuse and misuse). The programmes are based on scientifically proven current findings (medical evidence).
We offer these programmes for:
- Bronchial asthma
- Chronic obstructive pulmonary disease (COPD)
- Diabetes mellitus type 2
- Coronary heart disease (CHD)
